I have lost my certification card.  I certified with WASI/NASI 4 years ago in Utah.  How do I get another certification card?

Jim, in order to replace your WASI/NASE certification card you can contact them by phone at 1-801-363-9274 or visit their web site at www.naseworldwide.org. Click on the link for North America and you will see a link to the replacement certification card form which is a PDF download. You will be directed to mail the completed form, a photo of yourself, and the processing fees to:

Whenever a diver loses a certification card, I often encourage them to take advanced and specialty training if the diver hasn't already. If a diver has been an active open water diver for a few years then he or she can see the need for advanced training. Having an AOW card will allow a diver to discover greater opportunities when traveling and diving with resorts or charter boats. If a diver hasn't been diving actively, taking a refresher course and taking advanced or specialty training will serve to both sharpen and increase a diver's knowledge and skill base. Also, having AOW and specialty cards will allow you to keep them in different places when you travel so you won't lose your only card or all of your cards. Personally, I keep my instructor trainer, cave and mixed gas c-cards in my wallet and I keep additional instructor and cave cards in both my passport holder and log book. I normally keep the wallet in my pocket, my passport in my carry-on, and check my logbook with my gear. That way, I should be able to provide proof of diving certification if I were to have my wallet or passport stolen. That way I can at least go diving while trying to sort out the other matters of citizenship and credit card theft.

Around the house, if you have more than one c-card you can keep one in a safe place and carry the other on you in case you need it. If you're an active diver, having a c-card in your logbook, one in your wallet, one in your car and one in a safe place means you'll probably never be without one if needed. If any are lost then there's no stress while waiting for a replacement.
