Numb Ankle & Hyperthermia?

Question
QUESTION: hi i am male 42 years and i have been running a lot the past 4 years
i did run a marathon 4 months ago but only as a distance not
as a race . am far away to be in an athletic shape .
after that i felt i am pushing my self too far, so the past 2 months i reduced my distance to a max of 8 miles, 15 kilometers
i run those easily . 3 times per week . that is one day on one day off . holiday at weekends .
last month i did a stupid mistake . i went running repeatedly
at about 1 pm in the sun and i live in Egypt the temperature outside was more
than 40c in the shade . dunno why i did it . i was careless i guess
and i thought it will make me lose more weight .
it was easy and everything looked ok . but i guess i got
my self a Hyperthermia because i started feeling some
congestion in the heart with some headache and tone in my ears
while running my left leg started to feel numb below and around ankle and i was tired .i kept going then i took a rest for a week then i ran again for a week after 4 pm but the numbness continued i could feel it a little bit in my right leg but it gose away .
now after the rest i dont feel tired i feel great and i could
run easily the same as ever and as easy as ever but the numbness
in my left leg ankle did not go away . i dont feel it all the
time only with effort . its not strong and it gose away with
5 min rest , but still feels like a nail in the bottom of my foot from the front of it .
my question is should i do anything about it ? and i want
to go back to running is that ok ?
also i was thinking of asking a neurologist for the Hyperthermia
but i didnt find on this web . or maybe its called something else ?
thanks
btw i have some reasonable pain in my left varicocele
at times could that be irritating some nerves ?
one thing i noticed today if i got angry or exited i get
the numb feeling again
ANSWER: Dear Anas,

For your numb ankle.....
It is unrelated to hyperthermia. It is probably irritation along the path of the nerve to the ankle. The varicocoele may add additional pressure to the nerve. Anger produces tension-which tightens muscles and may irritate the nerve.
You probably had heat exhaustion-not hyperthermia. Hyperthermia is a serious condition & usually requires medical treatment.
Neither produce pain or numbness in one foot & ankle.
As far as the sharp pain in your foot-sounds like an irritation of one of the metatarsal heads.
Go see a neurologist for the ankle, a podiatrist or orthopedist for the foot & possible diagnosis of the ankle. Here in the U.S. a Board Certified Podiatrist should be able to diagnose the ankle problem-but may not be allowed to treat according to the state. In Illinois, we are able to treat ankles.
Good Luck!

QUESTION: thanks Dr.George. my actual concern was the heart and brain , i was
worried i was effected by the over heat . but now am not so worried
to run . actually while you went to vacation i went back running and
i was fine . the numbness was less and less and the sharp pain in the
foot front is almost gone , and i just feels some picking at times now.
i think the rest and go strategy is working with me . i did not check
at the hospital tho . i hop its safe for me to keep running without
doing that check up . am not lazy of afraid but i just feel fine
and running well i dont feel that i need treatment really .
i just needed to be sure am not doing something bad to harm my self .
i have responsibilities and i cant take chances . anyway i also checked
with Frank J. Pearl, M.D. for the heart and he told me its ok and
it was a pinched nerve .

Dear Anas,

It can take many weeks (4-12) to recover from a sports injury. Especially if you are still doing the sport that caused the problem.
Do not run again in those conditions & distance.
I used to to do full contact martial arts. Even though I feel like Bruce Lee-I know I am not--and I have family & business responsibilities---and even though I exercise hard- not to the previous levels.
If you checked online for your heart-then go see a doctor for a confirmed diagnosis. Can't examine a patient online for heart or brain problems.
The brain is probably OK. Can't comment on the heart.
