IF THE OPPOSING TEAM SUBSTITUTES A PLAYER, AND DID NOT TELL THE UMPIRE OR THE OTHER TEAM, WHAT IS THE RULE FOR THIS? THIS IS IN MIDDLE SCHOOL SOFTABLL.

For middle school softball you use the same rules as high school unless modified by your state association. There may also be some conference adjustments as well. I doubt they would have any changes to the substitution rules.
What you have described is an unreported substitution not an illegal substitution. If she would have been legal with proper notification to the umpire, the umpire simply makes the changes to his line-up card and issues a warning to the offending team. If the substitution was also illegal the rules regarding illegal substitution are enforced.
For a second offense, the offending player and the coach are restricted to the bench. A player with a legal ability to substitute, or the original player if they can re-enter, is put in the game. No outs are recorded unless the substitute was illegal as well.
