Question
High school player pinch-hits in the fourth inning. Starter then re-enters in the fifth inning. In the seventh inning, coach wants to re-enter his pinch hitter. The girl puts on a helmet and grabs a bat but doesn't make it to the batters box. Ump declares illegal substitution and an out. Ump was right or wrong?
Another question: Catcher fields a bunt and overthrows first. Runner makes the turn and heads for second. The ball continues rolling and goes out of play. Ump awards runner third base. Ump right or wrong?

Any player may be withdrawn from the game and re-entered once. As long as we are talking about batting for the same girl again, no problem.
When a sub enters the game and then is withdrawn, she can only re-enter in the same position in the batting order.
On a thrown ball, except a pitch, that enters dead ball territory (DBT), the ball is called dead when it enters DBT. A two base award is made to all runners. The two base award is made from where the runner was when the ball was thrown, not when it enters DBT. When this happens with a pitch, the award is one base.
I don' have quite enough information in either question to tell you if the call was correct. Post me a follow-up and let me know if the calls were correct based on my response to your post.
