I could right a book here on "all aspects" of improving your game.
I suggest you get with you past coach and sit down with him. Together wright down all the strengths and weaknesses you have.
Then set measurable goals for you to hit. I have had glandular fever too, it took away a big part of my season in my final year so I can emphasis with you. The big thing to work on is strength and fitness. Be sure not to over train and get plenty of rest. Put yourself to bed as a good hour and eat well, drink a lot of water.
Do not be too intimidated buy moving up a level in play. Get strong and fit and work on and skills that have slipped (like your through in)
Don't over think this, you will need to focus on your health and fitness first. Play the game and let your experience help you decide what to work on, don't assume anything, you could be selling yourself short.
Above all have fun.
