Can you please tell me the responsibilities of a middle linebacker in a 4-3 defense, with "Cover 3" formation?  

Linebackers do whatever you tell them to do. You can make your middle linebacker responsible for all inside runs, back side runs, or just flow to the ball. It's up to you. Most inside linebackers are told to read inside first before they move towards the ball.
