missing player from line up card

8 under boys--my son's team was home team and coach neglected to put a child on line up card.This was not caught or challenged until 3 or 4 batters later. The umpire called him out and took 2 of our runs back. I thought if the next batter got up to the plate and rec'd a pitch, there was nothing that could done until the next time around.

Berry comes to bat, but it was Adam's turn. Berry walks, then Carl comes to bat. Carl swings and misses the first pitch... Carl now becomes the legal batter, and nothing can be done.
However, I am a little confused. Was the player who came to bat currently in the lineup, or was he on the bench, and went to bat by mistake. In this instance, it would be an unannounced substitution, with no penalty.
