Scoring a Pinch Hitter

Player A has a one-ball, one-strike count and is lifted for a pinch hitter player B who singles. How is this scored in the book for player A and player B. Does A get an "at bat"? Does B get an "at bat" also as well as the single? How is this all marked in the scorebook?

Player A does not get an at bat. B does get an at bat and a single. Player A is shown in the book as in the game. The only way a player who doesn't finish an at bat, is if they have two strikes on them when they are taken out. If the pinch hitter makes an out when starting with two strikes, then the at bat is given to the first pinch hitter.
