When was the American League rule changed that once the second visit to the mound in a GAME the pitcher had to be removed changed that following the second visit to the mound in a GAME the pitcher had to be removed The National League had after two visits in an INNING. Now its the same?

If I understand your question correctly, I believe you're a little confused. The rule, for as long as I can remember, stipulates that a pitcher must be removed if visited for a second time during an inning, not a game. Both leagues have had the same rules in regards to this situation, thoughout the history of baseball, as far as I know (I could be wrong).
