What constitutes a visit "at the mound"? Is it crossing the foul line? Is it any step out of the dugout? If one manager calls timeout and the other one walks out about three feet out of the dugout and says something to his pitcher is that consider a visit?

Quite frankly, it depends on the umpire. I have seen some umpires charge a visit when a coach shouts instructions to the pitcher from in the dugout. I really have. The home plate umpire determines and designates what is a visit and what is not. I believe the rule is an actual visit to the mound which would mean to me crossing the foul line, but some umpires like to interpret and enforce rules at their will. While most umpires are great, some are on a power and ego trip.
